Yep, sorry have to do some juggling today. The email workload is higher than normal today. I should be able to setup a similar environment that you have setup on your site to figure out the 3rd factor in the equation tomorrow. Logically it appears you need 3 separate solutions, but the last piece of the puzzle does not have any Security Log entries/clues to work with.
